I give permission for the London Borough of Barking & Dagenham to collect and store personal data about me and the activities which I participate in for monitoring purposes on behalf of the local authority. I give my permission to be contacted using the details I have supplied, this includes phone calls, texting and e-mails. I understand I will be contacted about the project(s) I have supplied my details to, and that my information will not be shared with anyone else without my permission, unless it is a safeguarding issue and it is done for my protection. I consent to receiving phone calls and texts about events and sessions that are coming up and I give my permission for e-mails about events and opportunities to be sent to me via the Participation, Opportunity and Wellbeing service. I understand that the data may be shared with other local authorities and partner organisations in order to support me. I understand that the data will be held in line with the General Data Protection Regulations 2018 and will be kept for two years. This consent form allows your child to participate in the project, as described below. The information you provide in this form will be used for emergency purposes, whilst on trips and visits and if we need to contact you for any reason relating to your child’s participation in the named project. Your information will not be shared with other organisations or agencies and will remain on our system for the duration of your child’s participation.
By completing and signing this form you are agreeing to allow your child to participate in the project. You are also agreeing to allow us to store information about yourself (and other people you include as emergency contacts) and your child. All information will be stored in a safe place, either electronically or in paper form, in line with the General Data Protection Regulations 2018.

Name of group / project: Flipside
Venue: Regular sessions held at Vibe Youth Centre and Various venues for trips/visits. From January 2021 to March 2023.
Flipside would like to include your mobile number in a WhatsApp group that includes all other group members. The purpose of this is to enable us to communicate with you for free and keep everyone up to date with what’s happening. Communication from us will include reminders about sessions/events/meetings or things that you need to bring to sessions.
All members of each group will be able to view your number, so it is important to receive your consent before adding you to the group. Ground rules regarding this will be discussed in our group sessions, so everyone knows what is and isn’t appropriate. Any inappropriate behaviour will result in you being removed from the Whats App group to ensure that everyone feels safe and comfortable to use it. Youth workers will support you if you are experiencing problems.
I consent to my mobile number being added to Flipside WhatsApp group for the duration of my time in the group. I understand my number will be accessible to everyone else in the group and that I can request to withdraw my number at any time.
