If someone else is completing the application for you, they must complete the Declaration at the end of the application.

Finally, please be aware that this form saves the information you have entered onto it as you go along when you click on the 'Save and Next' button at the bottom of each page. Then use the menu at the top left to get back to the page you were completing so you can finish this off.
This means that you can complete separate sections of the form and come back and complete others later. For example, when you have collected the information you need to answer particular questions. The form, however, is set to time out after 15 minutes.
