| PCN Mentoring Project |
One to one mentoring support for YP aged between 11-18 years old. Providing tailored one-to-one support for young people (YP) across East London focusing on supporting mental health, wellbeing, reduction in negative/violent behavior, understanding their vulnerabilities, future aspirations and goals, and physical health improvement.
Currently two Primary Care Networks (PCN’s) in B&D – North and New West. YP need to be registered at the following surgeries:

Sessions are weekly, 60 mins long. They can take place in the Doctor Surgery, School/College or the Stadium in Stratford. Can also take place in other public spaces such as Libraries if suitable. | West Ham United Foundation |
